Brainstem Evoked Response Audiometry (BERA) is an objective and non-invasive hearing assessment used to evaluate how sound signals travel from the ear to the auditory nerve and brainstem. It records the electrical activity generated by the auditory pathway in response to carefully presented sound stimuli.
During the test, small electrodes are placed on the scalp and behind the ears to detect the brain’s electrical responses. Sounds, usually clicks or tone bursts, are delivered through earphones or insert earphones, and the resulting responses are recorded and analyzed by specialized equipment.
How BERA Testing Works
BERA does not require the patient to actively respond to the sounds, making it especially useful for infants, young children, and individuals who may not be able to participate reliably in conventional hearing tests.
The patient is asked to remain relaxed and still, preferably while sleeping. For young children, the test may be performed while they are naturally asleep; sedation or anesthesia may occasionally be considered when necessary under appropriate medical supervision.
The recorded wave patterns help the audiologist assess the timing and strength of neural responses as sound travels through the auditory pathway. BERA results can provide valuable information about:
- Hearing sensitivity and estimated hearing thresholds.
- The function of the auditory nerve and brainstem pathways.
- Possible conductive or sensorineural hearing difficulties.
- The severity and nature of hearing impairment.
- Hearing assessment in children who cannot complete conventional audiometry.
Importance of BERA for Children
Hearing difficulties in children can sometimes be difficult to identify at an early age. Undetected hearing loss may affect speech and language development, learning, communication, and social interaction. Early and accurate hearing assessment can therefore play an important role in timely intervention.
BERA is particularly valuable when behavioral hearing tests cannot provide reliable results. However, BERA should not be considered a replacement for a complete audiological evaluation. The results are best interpreted alongside other appropriate hearing assessments and clinical findings.
